FAQs and Facts

What is the application process for The Medallion?
After touring our community, an apartment may be selected. A community fee is required to reserve the apartment. The community fee will hold the apartment for thirty days.

The next step is an assessment by the Director of Resident Care. The Medallion strives to provide every resident with the services they need, and this assessment ensures that we can provide the services that the resident needs.

Additionally, the prospective resident’s physician will complete a form that provides our team with the resident’s health history. The physician may be an attending physician at a skilled nursing facility.

Both the assessment described above, and the physician’s form must be completed within 30 days of move-in.

Once we have completed the assessment and have the physician’s statement, The Medallion offers individualized admission forms and a contract for residence.

The following documents need to be provided:

  • State Identification Card
  • Social Security Card
  • Medicare Card and other Insurance
  • Covid Vaccination Cards
  • Copies of any advanced directives.

A check for the balance of the month’s rent will be expected. This amount is prorated.

Volunteers are like family, and we appreciate those who graciously support our resident activities. Volunteers brighten our community and most importantly enhance our resident’s quality of life. The Medallion has a wide variety of volunteer opportunities for people of all ages. Volunteers assist staff and residents in projects, tasks, and daily activities.

Adult Volunteers:

Whatever your interests or skills are we look forward to finding meaningful ways to put you to work. Whether you are helping with recreational activities, teaching arts or crafts, providing companionship, or getting to know our residents, volunteering is a richly rewarding experience for those who spend time at The Medallion.

Intergenerational Volunteers:

Intergenerational programs are very much appreciated. The Medallion residents enjoy meeting students and supporting their community services requirements for school. We encourage students to find opportunities to participate at The Medallion for their Bar and Bat Mitzvah projects, summer holiday, spring, and winter-break community engagement programs. The Medallion offers after-school and weekend activities that may be interesting for students. Let us know what your interests are, and we will assist with scheduling your activities, program, or project.
